    Alzheimer’s Disease – How to Spot the Signs

    Alzheimer’s disease is not normal forgetfulness as we age. Instead, it is a specific form of mental decline. And according to the Alzheimer’s Association it accounts for nearly 80 percent all dementia cases. Natasa Dragicevic, MD, PhD., behavioral neurologist and Alzheimer’s disease specialist with Renown Institute for Neurosciences, weighs in on diagnosing it and the importance of early medical action. How to Diagnose Alzheimer’s Disease In general, the signs of Alzheimer’s disease occur slowly, getting worse over time. For example, forgetfulness is a daily search – for shoes, keys and other misplaced items. Not only is memory affected, but also speech patterns and behavior. There is no single test for Alzheimer’s disease. “Specifically, a neurologist should be the one to diagnose Alzheimer’s disease given differences in presentation,” clarifies Dr. Dragicevic. “And ideally a behavioral neurologist (Alzheimer’s sub-specialist) will be managing the treatment,” she adds. Brain Imaging Diagnosing Alzheimer’s disease involves multiple approaches and medical providers. In short, medical history is reviewed along with a physical exam, lab tests and other diagnostic testing. “A medical workup includes a variety of tests. These include MRI and other brain imaging, as well as neurological and psychological testing. Furthermore, a lumbar puncture is performed to look for markers of the disease,” she states. What Causes Alzheimer’s disease? Although no one knows the cause, researchers think many factors play a role. Uncontrollable risk factors include your genetics and having a family member with the disease. However, the controllable risk factors include: reducing the risk of head injury and keeping your heart healthy. It’s important to realize that high blood pressure, high cholesterol, stroke and diabetes play a role in brain health. Blood loss to the brain causes vascular dementia, leading to long-term blood vessel damage. Symptoms of Alzheimer’s disease Generally speaking, the signs of this disease differ in each person. Yet noticeable behaviors include: • Losing the way to familiar places • Forgetting to pay bills • Trouble finding the right words when talking • Repeating questions • General confusion • Social withdrawal Alzheimer’s Disease – Benefits of Early Diagnosis Equally important, spotting Alzheimer’s disease early allows more time to benefit from medications and possible clinical trials. Likewise, nutrition and exercise changes can be made, increasing blood flow to the body, and perhaps delaying symptoms. Early diagnosis also allows for personal health decisions and quality-of-life conversations to take place.   According to the Alzheimer’s Association, these benefits include: 1. Medical advantage 2. Emotional and social comfort 3. Time to plan ahead 4. Cost savings A Brain Supporting Lifestyle “At the present time, treatment is limited,” explains Dr. Dragicevic. “Usually Alzheimer’s is a progressive ongoing disease – any management at this time is purely symptomatic.” However, she states the following lifestyle changes can help support brain health: • New hobbies such as painting, pottery, music classes or learning a new language • Crosswords, puzzles and playing games, such as Scrabble • Brain challenging mobile apps, such as Luminosity • 30-45 minutes of mild to moderate physical activity per day, such as walking • Eating a Mediterranean diet (primarily plant based foods)

    SOGI – The Most Important Terms to Know

    SOGI stands for sexual orientation and gender identity. Being a SOGI-inclusive medical facility means regardless of our differences, we treat others with dignity and respect. Sean Savoy, Renown Health Manager of Spiritual Care, explains SOGI terms and their importance in a healthcare setting below. SOGI History Although the UN passed a historic resolution on SOGI in 2016, it is still a relatively new term for some. Above all, an inclusive environment where every patient feels safe and valued, improves the quality of care in our community. In reality, defining gender and sexuality can be confusing. However, the terms we use to define gender and sexuality are an important influence on how we relate to each other. Generally, "sex" refers to the biological differences between males and females. Still the term “sex” doesn’t fully capture the complex biological, anatomical and chromosomal variations that can occur. Accordingly, having only two (binary) options – biological male or biological female – might not describe what’s going on inside a person’s body. In essence, assigned sex (also called "biological sex) is given at birth based on medical factors. These include your hormones, chromosomes and genitals. Most people are assigned male or female, and this is what’s put on their birth certificate. When someone’s sexual and reproductive anatomy isn’t clearly female or male, that person may be described as “intersex.”  However, a person’s biological or assigned sex may be different from a person’s actual or perceived gender identity or expression. The word “gender,” therefore, is more difficult to define. It could refer to the gender or sex role society determines is acceptable, desirable or appropriate based on a person’s perceived sex. Likewise, it could refer to an individual’s own gender identity or preferred gender expression. What is Gender Identity? To summarize, gender identity is one's innermost concept of self as male, female, a blend of both or neither. To clarify - how individuals perceive themselves and what they call themselves. Of course sometimes a person's genetically assigned sex does not line up with their gender identity. These individuals might refer to themselves as transgender, non-binary, or gender-nonconforming, for example. Another key point is the process of gender transition. This specifically refers to more closely aligning your internal knowledge of gender with your outward appearance. According to the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (DSM), the term – which replaces Gender Identity Disorder – "is intended to better capture the experiences of affected children, adolescents, and adults." What is Sexual Orientation? Sexual orientation is an internal or enduring emotional, romantic or sexual attraction to other people. For example, you could be heterosexual (straight), gay, lesbian, bisexual or even pansexual. SOGI Terms to Know Below are some common SOGI terms according to the National LGBT Health Education Center and the Human Rights Campaign. The following list is not meant to represent every term (or definition) used by the LGBTQ+ community. LGBTQ+ Stands for lesbian, gay, bisexual, transgender and queer (or questioning). The plus encompasses other identities that fall under the queer umbrella with one common theme: they are not strictly straight or cisgender. Queer An umbrella term used by some to describe people who think of their sexual orientation or gender identity as outside of societal norms. Some people view the term “queer” as more fluid and inclusive than traditional SOGI identities. Due to its history as a derogatory term, the term is not always embraced or used by all members of the LGBTQ+ community. Sexual Orientation Terms Asexual (adj.) – Describes a person who experiences little or no sexual attraction to others. Asexuality is not the same as celibacy. Bisexual (adj.) – A sexual orientation that describes a person who is emotionally and sexually attracted to people of their own gender and people of other genders. Gay (adj.) – A sexual orientation that describes a person who is emotionally and sexually attracted to people of their own gender. It can be used regardless of gender identity, but is more commonly used to describe men attracted to other men. Heterosexual or Straight (adj.) – A sexual orientation that describes a person whose sexual or emotional attractions and behaviors focus exclusively or mainly on members of the opposite sex or gender identity. Lesbian (adj., noun) – A sexual orientation that describes a woman who is emotionally and sexually attracted to other women. Pansexual or Omnisexual (adj.) – A sexual orientation that describes a person who is sexually and emotionally attracted to people of any sex or gender identity. Pansexual people may refer to themselves as gender-blind, asserting that gender and sex are insignificant or irrelevant in determining whether they will be sexually attracted to others. Gender Identity Terms Bigender or Binary (adj.) – Describes a person whose gender identity is a combination of two genders. Cisgender (adj.) – A person whose gender identity aligns with the assigned sex at birth or biological sex. Gender non-conforming (adj.) – Describes a gender expression that differs from a given society’s norms for males and females. Gender transition (noun) - The process by which some people strive to more closely align their internal knowledge of gender with its outward appearance. Some people socially transition, whereby they might begin dressing, using names and pronouns and/or be socially recognized as another gender. Others undergo physical transitions in which they modify their bodies through medical interventions. Intersex (adj.) - A variation in sex characteristics including chromosomes, gonads, or genitals that do not allow an individual to be distinctly identified as male or female. Non-binary Alternate terms are gender queer and gender non-conforming. Transgender (adj.) – Describes a person whose gender identity and assigned sex at birth do not correspond. Also used as an umbrella term to include gender identities outside of male and female. Sometimes abbreviated as trans. SOGI Awareness Again, not all people use the above terms in the same way, so respect and sensitivity are key. And the healthcare setting is an especially vulnerable one. Maintaining the dignity and humanity of every individual during a medical interaction is essential. The U.S. Department of Health and Human Services requires all Electronic Health Record (EHR) systems be able to collect SOGI information from patients. This promotes better understanding of health treatment outcome disparities in order to reduce them.     Department Spotlight: Infection Prevention

    Hygiene in healthcare is an essential backbone to providing the safest, most optimal care to every patient. From everyday handwashing techniques and getting all necessary vaccinations to surveilling our health system before the onset of outbreaks, at Renown Health, creating and maintaining a low-infection environment is a group effort. From ensuring fundamental infection prevention practices are carried out to identifying process improvement opportunities, Renown’s Infection Prevention (IP) department is here to help care teams be successful in their role. This effort is led by members who are devoted in sharing evidence-based best practices to our patients, employees, healthcare organization partners and community. Making “Zero” A Reality Reducing healthcare-associated infections is possible when core infection prevention practices are incorporated into the daily care of our patients. These team members are dedicated to Renown’s commitment to creating a culture of Excellence, caring for our healthcare environment, and our care teams. “Often times, when asked what we do in our work it can be a challenge to answer because unpredictability is built into almost every day; and in that, we as a team must divide and conquer,” said Susanne James, Manager of Infection Prevention. Every day, each team member is assigned a day to perform required surveillance activities. Whether they are reviewing patients’ infections and isolation and evaluating lab orders for possible infectious processes or reporting communicable diseases to the local health authorities and the CDC, every Infection Preventionist participates in improvement projects and address issues as they arise. The Infection Prevention team plans, organizes, manages and evaluates the Infection Prevention Program and ensures IP policies reflect current best practice. They work closely with all departments at Renown in order to achieve high levels of standards that are implemented to reduce transmission of infection. “As a team, we perform ‘Infection Prevention Environment of Care Rounding’ on all units and departments to ensure our environment complies with regulatory requirements,” said Erika Clark, Operations Analyst at Renown Health. "We work together with Facilities and Site Practice Leaders to ensure a safe environment.” The pandemic challenged all Renown teams to find creative solutions for issues no one ever imagined. Our Infection Prevention department worked tirelessly alongside Renown's Hospital Incident Command Structure (HICS) and care teams in creating several lines of defense against COVID-19, including: Launching Renown's Alternate Care Site to meet the need for critical patients. Bringing on the new lab tests and specimen collection criteria. Managing demand for personal protective equipment (PPE). Bringing vaccines to staff and then to the community. Managing ill employees and our visitors. Keeping all locations informed on pandemic status and changes (which were occurring at breakneck speed). These initiatives enabled Renown's care teams to focus on providing the safest care possible to patients saving countless lives. This solidified the fact that together, the team can accomplish anything. “Infection Prevention is proud to be a valued and respected member of the care team,” said Susanne James. “We appreciate being a trusted resource and take this responsibility seriously as this enables us to provide staff the best possible tools to be successful.” Fight the Good Fight Against the Spread of Disease Educating patients and providers plays a huge part on the Infection Prevention stage. This team provides educational materials on a range of topics, some of the most important are, proper hand hygiene and the use of PPE. All these materials can be found throughout our health system and serve as an important reminder – infection prevention starts with all of us. “We partner with every aspect of the health system – environmental services, nutrition services, lab services, facilities, pharmacy, physical therapy and occupational therapy, respiratory care, patient access, security, nurses, providers and quality...just to name a few!” said Susanne James. Our Infection Prevention team also partners with Washoe County Health District and other public health agencies to report – and in turn, stop the spread of – diseases and exposures. These close partnerships poise our team to do what we do best: Fight the Good Fight. “I greatly appreciate everyone in the IP department for sharing their expertise as I learned a new position,” said Erika Clark. “I love working with my team and how closely we work with other departments to ensure excellence in all we do.” Our health system is ever-changing. But according to the experts in our Infection Prevention department, the three main ways to prevent illness and disease that remain the same time and time again are to: Clean your hands. Hand hygiene is the most effective way of preventing the spread of infections. The CDC recommends washing your hands with warm soap and water for 15-20 seconds or using an alcohol-based hand sanitizer. Clean your hands often and assist our patients to do the same. Learn expert hand-washing advice here. Get vaccinated. Immunizations are one of the top methods you can embrace the fight against preventable diseases, such as COVID-19, flu, measles, tetanus and more. Discover how you can get vaccinated with Renown. Keep germs from spreading. Practice good hygiene by covering your cough or sneeze or wearing a mask and sanitizing frequently touched spaces. Stay home if you feel ill to prevent the spread of illness and infection. Be sure to bring up any illness concerns with your primary care provider. “Every aspect of our health system has an infection prevention component, and we work daily to ensure our staff, patients and visitors are providing and receiving care free from potential harm or infections,” said Susanne James. “We are often the first call staff make when they have questions, concerns or just need someone to help improve process or outcomes.” Reveling with Renown Proudly defining themselves as a “small but mighty” team, the six-person Infection Prevention department moves mountains to ensure the safety of anyone who walks through Renown’s doors. The tight-knit nature of this team can be felt from miles away, and to them, being “small but mighty” is not a hindrance, as they have the support of the IP Champions and the entire health system.   “I originally came to Renown in 2018 as a nurse traveler, the facility and people I worked with immediately made an impact on me,” said Brian Stroud, Infection Preventionist at Renown Health. “My wife and I chose Renown and Reno due to the positive impact the people had on us. Everyone is welcoming, helpful and caring. Now, I am a full-time employee, and the team has made my transition into infection prevention trouble-free." Renown’s mission of providing a genuine difference for the health and wellbeing of the community is heightened and echoed by the Infection Prevention team. “I chose Renown because of the reputation Renown has earned in the community because of the excellent care and level of services provided,” said Shannon Oriola, Infection Preventionist at Renown Health. “I absolutely love this team and my Renown family!” “What brought me to Renown was the opportunity to acquire diverse skills which then motivated me to pursue my master’s degree,” added Paul DeLeon, Infection Preventionist at Renown Health. “What keeps me here is the people – Renown attracts hard working, ethical and caring people.” "My wife and I loved coming out to the Nevada mountains on vacations," added Russ Laarman, Infection Preventionist at Renown Health. "After being in Michigan for 47 years, we took the plunge and made NV our home. The experience has been great! I work with a great team that is very knowledgeable and works hard to ensure positive outcomes for our patients and colleagues at Renown.” Their pride shines through with their own individual accolades, both inside and outside of work. This past year: Shannon Oriola received a scholarship from the Association for Professionals in Infection Control and Epidemiology (APIC) for “improving outcomes through knowledge and practice,” which provided her with a $1,000 scholarship to attend the annual APIC conference in Indianapolis. Paul DeLeon earned his Certification in Infection Control and Prevention from APIC. Erika Clark completed Renown’s High Potential program, a six-month program that gives employees hands-on learning experience with several competencies, including emotional intelligence, effective communication, building successful relationships and organizational knowledge. Brian Stroud and his wife Charla became official Nevada residents. Russ Laarman scaled Mount Whitney. ...And the list keeps growing! Charged with the goal of keeping disease spread at bay, our Infection Prevention team remains vigilant and dedicated to their mission.     Department Spotlight Cardiac: Catheterization Lab

    February is American Heart Month, but our incredible heart heroes are here to serve you all year long. So please join us in honoring the Cardiac Cath Lab team at Renown Health! At the core of who we all are, we find the heart: one of the largest sources of life that gives us the blood and oxygen we need to live full and healthy lives. When the heart doesn’t function to the best of its ability, a delicate approach led by cardiac experts might be necessary. These individuals can not only save lives but also help facilitate a higher quality of life.  The team entrusted to protect that vital source of life includes the hard-working and talented individuals in the Cardiac Catheterization Lab (a.k.a. “Cardiac Cath Lab” or “Cath Lab”) at Renown Health. These team members are known for their expertise, precision and quick-thinking in emergencies. When it comes to the function of your heart, you are in the best hands with this team. Their passion for Renown’s mission of making a genuine difference in the health and well-being of the people they serve is a testament to what they do every day.  Cardiac Champions  Whether the procedure is emergent or planned, the Cardiac Cath Lab wears many different hats. The team is a well-coordinated group of nurses, imaging professionals, electrophysiology technologists, interventional technologists and more who band together to provide a diverse array of services, including stent placements, pacemakers, balloon pumps, interventional angioplasties, TAVRs and of course, catheters.  “A day in the life in the Cath Lab can go various ways,” said Erica Drummer, RN at Renown Health. “Every day, you are either a part of the electrophysiology team, doing cardiac ablations and placing devises such as pacemakers; the structural heart team, which includes MitraClips for mitral regurgitation and TAVRs; or the heart catheterization team, which includes diagnostic angiograms, heart catheterizations, angioplasties and stents for clogged arteries.”  With the wide variety of cases the team sees daily, an average day in the Cath Lab consists of many moving parts.  “We handle cases from left and right heart catheterizations, where we can diagnose and fix the artery right then, to electrophysiology cases like heart ablations and pacemaker implants, all the way to emergency STEMI (heart attack) patients,” said Michael Blankenship, Electrophysiology Technologist at Renown Health.  “We do so many things in the Cath Lab,” added Laurel Douglas, RN, Imaging Manager at Renown Health. “We ensure we have 24/7 coverage for emergencies, as we often have to act at a moment’s notice to care for emergent situations.”  Many of those emergency situations involve heart catheterization.  If someone has a heart attack, the team immediately steps in to place stents to open the coronary arteries that are causing the attack. If a patient’s heart needs any assistance during an emergency, the team can place devices such as Impella (temporary ventricular support) or intra-aortic balloon pumps. Renown is a major advocate for advanced medical technology, and the Cardiac Cath Lab team embraces these technologies to perform life-saving procedures, all while working together as a cohesive unit.  “We work amongst each other to ensure sterility, safety and the best outcome for each patient,” said Zohra Benbrahim, RN at Renown Health. “We try to fit in as many cases as possible into the workday, and sometimes after hours, so patients can get the care they need.”  “Each team usually has 3-4 members, and each member has a critical job to carry out in order to implement excellent cardiac care,” added Erica Drummer, RN. “Many of our patients tell us that we all work well together, and I couldn’t agree more. We have become like a well-oiled machine or a NASCAR pit crew.”  Naturally, patients undergoing any heart procedure might feel frightened and anxious. Never fear – the compassionate Cardiac Cath Lab team is here to ease those fears throughout each patient’s entire visit. Their main solution? Communication.  “Being a patient in the Cath Lab can be daunting,” said Erica Drummer, RN. “When we have patients who may feel scared or have concerns, we try our best to connect with them, keep them informed and educate them on the procedure and the step-by-step process, including what they may feel before, during and after.”  “Overall, we try to keep spirits light,” said Zohra Benbrahim, RN. “Our team is excellent at explaining the procedure as it is happening. We also advocate for comfort throughout the procedure, which helps tremendously with anxiety, including giving patients warm blankets and music of their choice!”  Our experts in the cardiac Cath Lab make sure that no stone goes unturned when it comes to optimal heart function for every patient. Together, the team mends hearts and saves lives, day in, day out.  A Growth Mindset  Always learning, always improving, always reaching new heights – that's the Cardiac Cath Lab team.  This team’s accomplishments speak for themselves on both a local and national scale.  “Over the years, we have achieved several milestones, including becoming a STEMI Receiving Center, gaining our Chest Pain Center Accreditation (which is achieved by Cath Labs who provide emergent percutaneous coronary intervention for heart attack patients 24/7) and having our accredited electrophysiology program,” said Laurel Douglas, RN. “We also offer services that other facilities do not offer. We have brought in several new procedures to the Cath Lab over the years. This is impactful, as the community members get to stay here for treatment.”  The Cath Lab team is anything but ordinary. There is never a dull moment in this department, and despite an ever-growing caseload, the team is always ready to rise to the challenge and continue pushing to surpass the average.  “I am so proud of my team for providing care for such sick patients,” said Zohra Benbrahim, RN. “We never really know how the day is going to go because emergencies happen, and things always change. Recently, we had a record high volume of cases, and my team powered through these busy weeks like champs.”  “We strive to get our STEMI door-to-balloon times at or below 60 minutes,” added Erica Drummer, RN. “Door-to-balloon time refers to the time from when a patient arrives in the ER to the time the Cath Lab has a catheter guide wire crossing the culprit lesion in the coronary artery that is causing the heart attack. The national average (and current guideline) is 90 minutes, so our 60-minute time is a notable accomplishment for us.”  The importance of staying on top of your heart health comes center stage during American Heart Month, which is a prime opportunity for our cardiology professionals to educate the community and help them grow their cardiac knowledge. The cardiac quality team participates in community events throughout the month (and beyond!) to raise awareness for cardiac health. The team also outreaches to other health systems and healthcare partners to highlight what Renown has to offer in the way of heart care. Each Cath Lab team member always reminds patients about lifestyle choices and diet changes that can improve cardiovascular health, and they take this outreach with them all around Renown Regional Medical Center as well, walking through the hallways promoting many different ways of living a heart-healthy life.  The journey continues! The team will soon welcome a new Cardiac Cath Lab at Renown South Meadows Medical Center. The new lab will allow our experts in heart care to treat patients at our South Meadows location experiencing a heart attack and needing immediate intervention to open vessels in their hearts. According to the members of this department, being a quick-thinking, dependable team member is key to predicting a successful Cath Lab professional.  “A good coworker communicates with everyone and is able to be flexible when things get hectic,” said Zohra Benbrahim, RN. “Our team stays so close with each other because of everyone’s ability to recognize where help is needed. Especially in the Cath Lab, it is important to be always aware of the patient’s vitals and clinical presentation so you can speak up if you notice changes. There are always new procedures or products coming out that become integrated into our practices, so it’s also good to welcome new policies and be adaptive.”  “We are a small team, and everyone sees each other as a friend,” added Laurel Douglas, RN. “We all know we will need help at some point, whether it be covering an on-call shift or helping during a procedure, so we all help each other. Teamwork and positive attitude are really what makes you a good team member.”  Not only are these individuals dedicated to impacting the lives of patients with the very organ that beats life into them, but they also are devoted to Renown – and it shows.  “I started working for Renown when I was 19 years old, and Renown was the medical system I went to for care; as I spent more time working here and becoming a part of the community, I could not see myself working anywhere else or getting care at any other hospital,” said Zohra Benbrahim, RN. “I truly believe Renown provides the best care and best serves the growing community of northern Nevada.”  The Cath Lab team members agree that Renown is a great place to both start and continue your career. Many of them have taken advantage of Renown’s training and educational opportunities, which in turn have helped them expand their knowledge to better serve the community.  “I chose to work at Renown because it gave me the place to grow in my career and experience different avenues within my career,” said Michael Blankenship. “I first started off as a student here, and I quickly realized that everybody I worked with genuinely cares for our patients. I started off in x-ray seeing patients and eventually did rotations up in the operating room, and I got hired full time in the Cath Lab.  After a short time being here, I was crossed trained in the electrophysiology lab, where I furthered my knowledge in my career.”  This dynamic department is actively hiring, and many open positions are sign-on bonus eligible. The team cannot wait to welcome you to their, as Erica Drummer puts it, “well-oiled machine.”  “Being a Cath Lab nurse is very rewarding, especially when you’re able to be a part of a team that can save someone’s life,” Erica closes.

The Surgical Preadmission department (a.k.a. “preadmit”) at Renown Regional Medical Center and Renown South Meadows Medical Center is a dynamic and compassionate group of nurses, case managers, chart managers and more who are dedicated to guiding each patient through the surgical process. With extensive knowledge and expertise under their belt, the teams are equipped to make a genuine difference in the health and well-being of all patients, for all surgeries, at all times of the year. Surgery Starting Ground The Surgical Preadmission teams are crucial aspects in the successful outcome of every surgical procedure. Comprised of skilled healthcare professionals, this department is dedicated to providing comprehensive support and care before heading into the pre-operating room. “Our job is to prepare every patient for surgery, make sure all their pre-surgery testing is done, ensure they understand their fasting and medication instructions, have had their questions answered, have a ride home and know what to expect during surgery and after so their recovery can go smoothly and without complications,” said Debra Bennett, RN, Supervisor of Surgical Preadmission at Renown Regional. “Each patient is unique, so each experience is different.” Our preadmit nurses are the masters of communication, directing thorough assessments – including medications, tests and clearances – and addressing any questions or concerns patients may have, never missing the opportunity to inform them of exactly what they will expect in surgery. “I do a complete history on every patient while giving them detailed pre-operating instructions and helping them answer any questions to the best of my ability,” said Nancy Hilts, Surgical Preadmission RN at Renown Regional. “I am proud to be able to help allay their fears and concerns using my 30 years of pre-op experience. I offer an avenue for them to feel comfortable opening up to me.” “We always tell patients that they have great surgeons and a fantastic team that will be watching over them and taking care of them every step of the way,” added Jon Capallupo, Surgical Preadmission RN at Renown South Meadows. “We also give them plenty of educational handouts and video content, in addition to verbal instruction, to ensure they are as prepared as possible.” The nurses then pass the reigns onto the chart managers, who prepare the charts for surgery and ensure all documentation is up-to-date before sending them to the pre-operative team. The expert surgical case managers also step in to prepare a thorough discharge plan, along with reviewing pre-operation orders for status, consent, codes and more. The team does several of these initial visits virtually, and they are looking forward to soon phasing all preadmit case manager preliminary visits into a virtual model. From assisting the pre-op and post-anesthesia care unit (PACU) teams with discharge planning concerns to helping on the outpatient side with anything from oxygen equipment and catheters to transportation issues and those experiencing homelessness, our pre-admit case managers are always up to the challenge. "We are true patient advocates, alleviating concerns and fears along the way,” said Mary Carl, RN, Case Manager at Renown Regional. “Just to name a few things we do on a daily basis, we see our total joint and non-weight bearing patients during their preadmission appointment, so they are aware of the medical equipment they need and if it is covered by insurance; provide education for Aspira catheters and gastrostomy tubes; advocate to ensure tube feeding and dressing changes are set up for hospice and home health patients; and escalate concerns of patient safety to ensure a safe discharge.” In the midst of it all, there is never a dull moment in the preadmit teams. At Renown Regional alone, the preadmit professionals see more than 1,600 patients a month – and each one has a hyper-personalized experience with their very own preadmit team. “Many times, we are the first impression a patient has on our organization; after all, we touch more than 97 percent of patients that are scheduled for surgery, cardiac catheterization lab procedures or interventional radiology,” said Amy Schler, Surgical Preadmission RN at Renown Regional. “We also collaborate with many other departments in the hospital as well, from surgeons and anesthesiologists to case managers and nurse navigators. Our work in preadmit impacts the entire surgical process.” Holding a diverse array of experiences, our preadmit department plays an integral role in our commitment to providing the highest quality of care to every patient. Knowledge is Power Heading into surgery feeling fearful and worried is normal for any patient. However, how would you feel knowing that each individual member on your preadmit team has, on average, 23-25 years of experience in the field? This is the reality for our Surgical Preadmission department – and they put that vast knowledge to great use to bring a sense of calm to patients. “Our team members have worked in various departments within our organization, and they bring a wealth of knowledge that we share with each other, and most importantly, with our patients,” said Amy Schler, RN. “If you have hundreds of years of nursing experience, you can speak not only as a nurse but also as a patient. It allows you to give a more personal perspective on what patients can expect in their recovery. Being able to assess their emotions and provide feedback to our pre-op and PACU teams help the patient have a better experience.” “Many of our nurses have close to 40 years of experience each, and they have an extremely large knowledge base since we see patients from newborn to geriatric and from easy procedures to complex surgeries,” added Debra Bennett, RN. “Another great aspect of our team is the varied nursing backgrounds we all hold – surgery, pre- and post-op, labor and delivery, pediatric intensive care, cardiology, gastroenterology, urology, cardiac cath lab, home health and everything in between. Communication between departments is so important!” All members in this department, regardless of which clinical area they came from, surgical or non-surgical, can easily translate their skills into the work they do in preadmit – and they only continue to grow. “As a surgical preadmission nurse, I have used my years of experience as a nurse in surgical services,” said Terri Delatorre, Surgical Preadmission RN at Renown Regional. “I started as a floor nurse with orthopedics for 12 years, and then I worked with the PACU for 11 years. This has helped me give great understanding and care to our surgical patients.” “Because we have staff with such a vast knowledge base, we can rely on our years of working within our organization to help alleviate fears that the patient may have,” added Amy Schler, RN. We can prepare them for what to expect in pre-op and PACU and educate them on what to expect post-op, including any barriers they may face. For example, mastectomy patients may not realize they will not be able to raise their arms for 7-10 days post-op, and total knee patients have to navigate stairs and housing access. Helping patients think about barriers at home that they may not have thought about helps them prepare prior to surgery, enhances their healing and provides a better surgical experience.” The preadmit team works closely with our best-in-class surgeons and anesthesiologists, continuing to grow their expertise along the way while learning alongside our talented providers. For instance, when it comes to our Renown South Meadows preadmit department, anesthesiologist Nariman Rahimzadeh, MD provides excellent guidance for the entire team on state-of-the-art anesthesiology practices. “I am very proud of the work we do with Dr. Rahimzadeh,” said Lisa Closson, Surgical Preadmission RN at Renown South Meadows. “Together, we ensure patients are safe for both surgery and anesthesia.” Despite the challenges that come their way – whether it be changes to process and workflow to navigating support for patients after they leave the surgery floor – the preadmit team cleverly uses their collective wealth of knowledge to bring hope and comfort to all patients. “Our nurses are such warm, caring and compassionate humans that do their best to ease any fears and anxieties patients may have,” said Debra Bennett, RN. The Pride of Preadmission The pride of our preadmit team lies in their ability to make a positive impact on all patients they serve. To them, their work is not just a job – it's a calling. And they do it all while working together to elevate their team and performance. “Our team is most proud of the quality of care we provide to our patients and our abilities to troubleshoot difficult situations to ensure they have a great surgical experience here at Renown,” said Mary Carl, RN. The entire department supports each other by working collaboratively and relying on each other’s expertise to provide the best possible care for patients. They understand that their success as a team depends on their abilities to support and help each other. “Our team is awesome here at South Meadows,” said Jon Capallupo, RN. “We can turn to each other for support, and we all work very well with each other. I am glad to be a part of this team.” “I am proud of how well all of us in preadmit works with each other every day,” added Lisa Closson, RN. “We try to make patients feel comfortable from the moment they arrive to the time they leave the department.” The pride that our preadmit team expends goes beyond their departmental limits – these team members are also trusted teachers. They work closely with cancer nurse navigators to teach weekly classes for patients who have been newly diagnosed with breast cancer, coupled with lymphedema prevention and education classes. The team also encourages participation in Renown’s free smoking cessation programs to help their patients learn the risks and benefits of quitting smoking. When not serving patients or teaching classes, you can find many of these team members taking charge of multidisciplinary committees within our health system, including breast leadership, gastroenterology leadership, shared governance, infection control and recruitment and retention. On top of it all, this team certainly knows how to celebrate, with several of their members being a part of their own department-wide Celebration Committee, where they gather for retirement parties, baby showers and team get-togethers. Always active and never passing up a challenge, our preadmit department are shining examples of Renown’s Culture Commitments, especially Caring and Collaboration, and the pride in the vital work they do every day is limitless. “I am so happy my position in preadmit opened up for me at a time when I was really feeling challenged in my career,” said Nancy Hilts, RN. “The team that we have here is amazing.     Staying Heart-Healthy with Genetic Screening

    February is American Heart Month. While cardiac care is crucial every time of year – especially as heart disease stays the number one killer in the United States – American Heart Month serves as a great reminder to stay on top of your heart health. We spoke with Malina Ruiz, a cardiology nurse practitioner at Renown Health, on three key ways to embrace heart-healthy living and how genetic screening can help inform you of certain genetic risks that can play a role on the cardiac life stage.  1.  Invest in a heart-healthy diet. Eating a diet that is rich in fruits, vegetables, whole grains and healthy fats (including monounsaturated fats such as avocados and polyunsaturated fats such as sunflower oil) is a key defense in protecting your cardiac health, according to Malina. While you are filling your plate with these nutritious foods, remember to keep an eye on your saturated and trans-fat intake, and try limiting foods that are high in those fats.   “No matter what age we are, maintaining a heart-healthy diet will always be important,” said Malina.    Need help finding cardiac-friendly meals? The National Heart, Lung and Blood Institute makes it easy with heart-healthy recipes and tips from experts.  2.  Do an exercise audit. “Keeping an active lifestyle during our younger years is one of the most important factors that affect heart health in future years,” said Malina. Maintaining a regular exercise regimen that allows you to raise your heart rate and break a sweat can help prevent future cardiac events.     A good rule of thumb is to aim for 150 minutes a week of moderate-intensity exercise, which averages out to 30 minutes a day on 5 days out of the week.    It’s never too late to start a regular exercise routine! Exercise doesn’t have to be something you dread – leverage American Heart Month to find activities that you enjoy. The American Heart Association can help you discover new ways to move your body. At the end of the day, as Malina emphasizes, “any movement is better than nothing!”  3. Don't skip those check-ups.     Department Spotlight: NICU Transport

    Bringing in new life is an exciting and beautiful occasion. The possibilities for the newest members of our world are endless – what will the child grow up to become? What will they achieve in their lifetime? Anyone will agree that a new life is precious and protecting that life by any means possible is a necessity.  Sometimes, however, a baby’s journey into our world brings along obstacles. Prematurity, congenital defects and other complications can accompany a birth and the baby’s first few moments of life. But what happens when these complications happen outside of a fully-functioning Labor & Delivery unit, or if the complications need a special level of intervention with a health system capable of caring for their unique needs?  Enter the NICU Transport team at Renown Health. As the only Neonatal Intensive Care Unit (NICU) Transport team in northern Nevada, these dedicated nurses, respiratory therapists and neonatal nurse practitioners – who have normal unit duties of their own at Renown Regional Medical Center – step up to the plate to take on this extra duty to support members of their community facing some of the scariest moments of their lives. To them, transporting babies in critical condition and giving them the best chance of life isn’t just a responsibility; it’s a calling.  The NICU on Wheels  When the Saint Mary’s maternity unit, which was Renown’s partner in NICU transport duties, made the decision to close, our dedicated NICU nurses and respiratory therapists diligently rose to the occasion to fill the demand. Dubbed the “NICU on Wheels,” Renown’s team went from taking on transport duties every other day to 365 days a year – and they are always ready to go at a moment’s notice, whether it’s by ambulance or by fixed-wing aircraft.  “When we learned about the closure we knew it was our calling and knew we had to step up,” said Rachel D., Neonatal Transport RN at Renown. “We still have regular days on the floor, and we each switch off being on-call for transport. Once we get a distress call, we have 30 minutes to get to the hospital, grab our gear and go. We have to essentially put a whole room’s worth of portable equipment in one bag.”  “I really enjoy being a part of the transport team and making a difference from northern Nevada to northern California,” added Sam V., Supervisor of Respiratory Care at Renown. “We are able to practice at the top of our skill level and use autonomy that not every practitioner gets to use.”  Each NICU Transport unit works in teams of three when heading out into the field: one nurse (our bedside experts), one nurse practitioner (our education experts) and one respiratory therapist (our lung and breathing experts). With several years – and in many cases, decades – of experience in NICU nursing, NICU transport duties and respiratory therapy, this team is poised to provide state-of-the-art lifesaving care en route to the hospital. This diverse array of experiences among the team especially comes in handy as they continue to face the increased transport workload.  “We are so thankful to have Saint Mary’s employees come over and work on our team after their maternity ward closed,” said Melyssa H., RN, NICU Transport Coordinator at Renown. “Helping the babies, as well as helping the scared families, will always be our top priority.”  “I love the complexity that babies provide in the NICU,” added Jennifer J., Neonatal Transport RN at Renown. “Providing hands-on care right at the bedside along other members of the team is such a rewarding experience.”  Education is also a crucial tool in NICU care, especially in the field. The nurse practitioners on this team make it their mission to outreach to the community, including our rural health partners, and help them build the tools and skills they need to care for our smallest and most critical patients.  “I have been on a few transports where we went out and did not have to bring the baby over to the hospital, and we were able to provide lots of education instead,” said Shiela A., Respiratory Specialist at Renown. “For instance, I was called out to Fallon for a baby that was in respiratory distress. When we arrived, we saw the baby was awake and active although intubated. The tube came out, and we retaped it. While we stayed to observe the baby to make sure everything was okay, the nurse practitioner on the team provided educational tools to the family on what they can improve on regarding their baby’s tubing. Our nurses do such a great job with these teachings.”  “When I started, I quickly noticed that more education was needed in the field,” added Jennifer J., RN. “Now I see the momentum we’ve had in these communities really take full force. Because of this education, and the confidence we instill in our patients’ families, each baby’s transition to our team has become so much easier. It has been awesome to watch this progress over the years.”  With immense experience, education and a strong commitment to their community under their wing, the NICU on Wheels is only just getting started.  Teamwork Makes the Dream Work  Running a successful NICU Transport operation, especially as the sole health system participating, takes a village. With only three professionals out in the field, the team leans on their fellow nurses, physicians, surgeons and specialists to make the operation run as smoothly as possible.  From REMSA and Care Flight helping load patients in and get them to the hospital safely, to Renown’s Clinical Engineering team who are first on the scene when machines need repairs and troubleshooting, everyone rises to the occasion to achieve the best-possible outcome for our youngest patients at some of the most challenging moments of the beginning of their lives – and they are fully trusted to move those mountains.  “Being a part of the NICU Transport team has given us the opportunity to facilitate a deeper, meaningful relationship with providers and a patient’s entire care team,” said Rachel D., RN. “They entrust us with our assessment skills, opinions and skill levels. There is an equal layer of trust between us and them.”  Despite the pressure they feel every day, the NICU Transport team never let it overtake them and instead let it empower them.  “We are under a lot of pressure and stress regardless of the case, and yet, we all work so well together; the teamwork really is incredible,” said Lisa A., Respiratory Specialist at Renown. “I am very proud of how our team steps up and is very intricate, even down to organization. For example, team members like Shiela arrange all the respiratory bags and make sure they are laid out in a way that makes the procedure as simple as possible to ensure there are no complications.”  The nurses and respiratory therapists also partner together on the NICU floor and are always ready to provide coverage when their transport colleagues get a call. Committed to collaboration and ready to jump into action, the NICU Transport team works together to elevate each other and the bottom line for each patient.  “This is a full-time gig, and the group works together to make sure everyone is covered; we are all very collaborative,” said Sarah K., Respiratory Specialist at Renown. “We all think of our patients as extra special, and it’s nice to know that you can make a difference in the baby’s life and the lives of the family. We all have a sense of purpose in giving back to the community.”  “I am just so thankful to be a part of this team,” added Marcia A., Neonatal Transport RN at Renown. “We really have developed a strong bond.”  A Track Record of Success  The life-saving measures that each member of the NICU Transport team take surpass hospital walls. The impact they have on each baby are remembered and celebrated for years, and they often develop lifelong relationships with patients and their families after their stay in the NICU.  One prime example of this can be found in an impassioned letter from Tess, the mother of a young boy who was in the midst of respiratory distress and needed immediate intervention. Because of the valiant efforts of Renown’s NICU Transport team, her child, Warren, is a healthy and happy baby to this day. She recalls the experience: Warren had stopped breathing while getting routine blood work. He was resuscitated twice while at an area hospital. We made the decision to transfer him to Renown for him to be admitted into the NICU. Within an hour and a half of this decision, Renown had sent a team consisting of a respiratory therapist, nurse and nurse practitioner to come and get him. As soon as the team entered the room, we immediately felt safe and comforted. The team made sure we knew exactly what was going on and what was going to happen in the following hours, and they let us know they would do everything for Warren. He was resuscitated two more times by the respiratory therapist while getting ready for transport. At that time, the decision was made to intubate for a smoother ride to Renown. We knew you all had his best interest in mind, and of course, we trusted you with everything. The intubation went well, and he was packed up and ready to drive. Today, Warren is hitting all of his developmental milestones and is a normal, happy baby in everyone’s eyes. We can’t thank the NICU Transport team enough for taking such good care of Warren and helping him Fight the Good Fight.  “This success wasn’t just due to our three-person unit – our manager, Jason, also jumped in and drove all the way to Carson City to stabilize the child,” said Shiela A. “This just goes to show that no matter our title, we are always here to help and make a huge difference in the lives of our patients.”  Success stories of children like Warren describe the common mission of the NICU Transport team in detail, and while there are more of these stories to come, the team is always ready to make the necessary sacrifices for the greater good of their patients.  “Sarah K. is like Warren’s guardian angel,” said Tess. “We call her ‘Auntie Sarah’ now. He wouldn’t be here if it weren’t for her and the entire NICU Transport team. It takes a special kind of person to work in the NICU. I can never repay this team for what they’ve done for Warren and our family.”  “I am proud of this team, their flexibility and the sacrifices they have made to do transport 365 days a year,” added Melyssa H., RN.”  The remarkable success of the NICU Transport team serves as proof of the unwavering and tireless commitment of each and every team member, bringing hope to families during their most difficult moments.  “NICU Transport forces you to be confident in yourself and your abilities and to push yourself a step further,” said Rachel D., RN. “I am a better nurse because I joined the transport team.”

    Department Spotlight: Child Life

    March is National Child Life Month. Join us in recognizing our Child Life team at Renown Health!  The hospital environment can be a daunting and confusing place for a child. Whether they are admitted to the hospital themselves or have a family member who is currently undergoing treatment, they undoubtedly bring along a lot of overwhelming questions and concerns.  No one would be better to guide them through the process than the Child Life department at Renown Health. Our devoted Child Life Specialists and Child Life Assistants dedicate themselves to helping our youngest patients and children of relatives in the hospital cope with their anxieties and fears. With the smiling faces of the Child Life team on their side, children at Renown have a sense of solace while they are in the hospital with a team to cheer them on every step of the way.  Offering a Hand to Hold  From the first moment of a child’s visit to Renown, they are never alone. The Child Life team, which is made up of three specialists and two assistants, is here to provide a helping hand throughout the entire hospital process, explaining each test, procedure and process every step of the way and creating the best possible plan for each patient – all while fostering and maintaining a safe and supportive environment.  “We are specifically trained to help patients and families throughout their healthcare experiences,” said Amanda Cleary, MS, CCLS, Child Life Specialist. “This looks different for every family. We are trained to tailor our services to meet individual family and cultural needs based on our assessment.”  “It does not take long for a Child Life Specialist to create a safe relationship with a pediatric patient,” added Angie McEvers, CCLS, CFLE, Child Life Specialist. “The Child Life department can help any care team do their job by redirecting a child’s focus and distracting them from painful interventions, and this is a win-win for both the patient and the care team.”  This team’s primary goal is to make talking to doctors and undergoing tests a little less intimidating for our youngest patients. Interaction is a crucial part of a child’s healthcare journey. Our Child Life Specialists are masters at procedure preparation, surgery preparation, trauma support and diagnosis education while meeting each child where they are at developmentally.  “A day in the life of a Child Life Specialist is never the same, and every child is different,” said Jennifer Nunzir, CCLS, Child Life Specialist. “To make sure patient care is possible, I make sure I am here mentally for the patients and families. I am ready with a smile to greet them and try to make them comfortable from the start. We are always present to support a patient and family with a new diagnosis, talk to children when there has been a tragic accident, make memories for families and giving children a distraction from tests and procedures.”  “One of the biggest benefits to our job is our ability to provide emotional support and communicate with families in a way that makes them feel seen and heard,” added Amanda Cleary. “We listen to the family’s past medical experiences and work on building rapport by building those connections. When you get a child to open up, you see who they are as an individual, and we build from there.”  The efforts of the Child Life team go beyond helping children with coping skills. They also jump in with activities to keep kids of all ages engaged – from arts and crafts to pet therapy.  “Bringing children smiles through play and giving them fun things to do – whether it’s watching a movie, playing a game, doing an art project, going for a wagon ride or taking them to the Children’s Healing Garden – makes me feel I am making a difference,” said Jennifer Nunzir. “If I can get the child and family to at least smile or laugh, I feel I have already eased some fear and anxiety. It truly is the best feeling to get them smiling and laughing.”  “Through play, conversation, distraction or relaxation techniques, patients cope better, and families are calmer,” added Angie McEvers.  Teaching Every Step of the Way  Education is a crucial aspect in this department. Each Child Life team member works closely with their patient’s care teams to break down “doctor speak” with age-appropriate explanations.   “Children come into the hospital for all sorts of reasons, from a broken bone to cancer; our background in child development and psychology helps us collaborate with the patients’ providers and explaining what each part of the care journey is depending on a child’s developmental level,” said Amanda Cleary. “For example, we can explain what a brain tumor is with playdough and help a child with a new diabetes diagnosis understand what it means to balance sugars using play food. It's all about providing family-centered support so that they feel more in control. I can’t tell you how many times I’ve seen parents’ eyes light up when the kids play. They are all coping, learning and growing together.”  To Help with the Happy and the Sad  When healthcare journeys don’t go as planned, support from care teams is crucial for families facing grief or loss. The Child Life Specialists are one of the first on the scene to provide support for children and their families, providing education on how to talk to children about death and helping families preserve the memories of their loved ones for years to come.  “One of the most difficult things we do is provide bereavement support,” said Angie McEvers. “Not only do we provide support for families of pediatric patients but also adult patients who have children. Our team takes the time to help families with memory making with handprints, journals, books, the list goes on. No one should have to go through this process alone, and we make sure that doesn’t happen.”  Above all else, the Child Life team are here, first and foremost, for the patients and their families. They are the drive that keeps them going – and helps them reinvigorate that passion for their work every day.  “I have met some of the most amazing children and families, and they definitely are what keep me here at Renown,” said Jennifer Nunzir.  The Road to Child Life  The journey to becoming a Child Life Specialist looks different for every team member, and many of them discovered the field while pursuing careers in other healthcare areas. Beyond their passion for children and families – and their Certified Child Life Specialist credential – our Child Life team members are also highly educated in the fields of human development, nursing and psychology, knowledge that benefits them greatly in their career.  Amanda Cleary had the unique experience of pursuing a Child Life career at Children’s Hospital of Los Angeles before she arrived at Renown last year, implementing her knowledge from her bachelor’s degree in human development and family studies into a clinical child life internship and a robust career in Child Life.  “While at Penn State, I learned about the Child Life career path through the Children’s Miracle Network Dance Marathon,” said Amanda. “I knew that this was the path I wanted to take. I did a 600-hour internship, and from there I was hired at Children’s Hospital of Los Angeles, where I stayed for 11 years. As a lifelong learner, I knew I wanted to pursue more – so I achieved my master’s degree online in child life leadership and advocacy at the Erickson Institute in Chicago, and what I learned in that program continues to serve me well in my career at Renown.”  Other Child Life Specialists, like Angie McEvers and Jennifer Nunzir, had already been with Renown for several years in other departments and built up a wealth of healthcare knowledge before discovering their passion for the mission of Renown’s Child Life program.  “I have been here for 23 years; I always knew I wanted to work with children in the hospital because I love kids, and although I originally wanted to be a nurse, I realized that I actually wanted to be on the other side of helping,” said Jennifer. “At the time, I didn’t know there was a job like the Child Life Specialist out there, and I feel so lucky to have found this career. Over the years, as Reno has grown, so has our program, which is very exciting. I knew Renown would be the place where I could grow as well."  “This year is my 24th year with Renown,” added Angie. “Before I worked here, I was an Advanced EMT/Firefighter while pursuing my nursing degree. In working with the medical team from Children's Hospital of Oakland in pediatric hematology/oncology, I was able to see first-hand the benefit of Child Life. I was driven to finish my human development degree and passed my certification testing, and ever since then, I’ve been with Pediatrics, PICU and the Children’s ER.”   We could not have said it better ourselves – this is a field that is constantly evolving and progressing, and while not everyone is familiar with it, our Child Life Specialists are ready to change that narrative.  “There is a lot of opportunity here to continue to grow the Child Life program,” said Amanda. “Not all hospitals have a Child Life program, but we are lucky that Renown does. Any aspiring Child Life Specialist should consider coming here, as it has been a great experience. It is a familial feeling here, and I definitely feel like I am in the right place.”  There is Strength in Numbers  It takes a village to serve the emotional needs of the many children and families that walk through the doors of Renown Children’s Hospital, and our Child Life Specialists are considered some of the key leaders of that village.   The outpouring of support for our Child Life program is astonishing. Not only have our patients and employees made generous donations, but members of the greater Reno-Sparks community are always there ensure kids in the hospital can be kids. The Nevada Wolf Pack, SCHEELS, Great Reno Balloon Race and Atlantis Casino are just a few of the many generous organizations that have donated both toys and time to our youngest patients.  To support any child dealing with a serious illness, especially cancer, Beads of Courage steps in to help children “visually record, tell and own their stories of courage” using beads as a symbol. Representatives from Beads of Courage volunteer their time to help our youngest patients keep track of their valiant fight against their illness, adding a bead to their necklaces after every step in their care journey.  “We rely on our generous community and donations that come in, and we see a lot during the holiday season – we definitely can benefit from this year-round,” said Amanda.  The donations allow the Child Life team to expand their play, coping and distraction opportunities for patients and families, allowing them to reach more children within their small, mighty and growing team of five.   “Our goal as a team is to work with as many pediatric patients and children of relatives seeking treatment as possible, so the opportunity to grow our team would be awesome,” said Amanda.  And grow they will! The team recently welcomed two dedicated Child Life Assistants, Marissa Arriaga and Sydnee Patterson, during Child Life Month. These team members work alongside the Child Life Specialists to help provide opportunities for developmental play, both in the playroom and at the bedside. Marissa and Sydnee ensure that the kids can still be kids, even while they are in the hospital.  Additionally, the Child Life department is currently hiring more Child Life Specialists.     Love Endures - A Remarkable Recovery Leads to a New Future

    It’s true that a road trip can change your life, and it did for Sergeant Brandon Ott, just not in the way he was expecting. For nine years, Brandon worked alongside his friend, Denton Tipler, at the police department in Florence, Oregon. To break the monotony of the COVID-19 lockdown, they planned a ‘guy’s trip’ - a pandemic safe, motorcycle adventure traveling through Idaho and Washington. On the morning of July 22, 2020, they set out on their journey. But by 8 p.m., they ran into rough weather - with wind, poor visibility and heavy rain as they rode into Nevada. Four miles from their destination, on a desolate stretch of Highway 140, a gust of wind hit Brandon. Denton watched in disbelief as his friend bounced, tumbled and ultimately came to a stop after sliding over 100 yards. He flagged down the next vehicle, a semi-truck, and the driver did traffic control while another passerby drove to get cell service and call 911.  A Distressing Call  About 10 p.m. the same night, Angie Brog, Brandon’s partner, picked up her phone at the Florence police station. A Nevada highway patrolman let her know Brandon was in a motorcycle accident and had been taken to a hospital in Winnemucca, NV. From there he took a life flight to Renown Regional Medical Center.  Angie immediately called Brandon’s parents and told Addison, their 9-year-old daughter, the news. They quickly packed their bags and drove to Nevada. On the drive to Reno, an ER doctor called Angie to give her Brandon’s status, “I can’t thank him enough. I was so worried. He even gave me his personal cell phone number.”   Upon arrival, she ran to see Brandon in the trauma ICU. “His face was shattered, he had a collapsed lung, a tube was down his throat and something was in his head to relieve pressure from his swelling brain,” she remembers. “He was not expected to live.”  While Brandon was in a coma for three days, Angie was by his side, trying to come to grips with a new normal. “The doctors did not sugarcoat it,” she recalls. “They told me to prepare myself if he pulled through.”  It was a rough week in the ICU with Brandon heavily sedated, so he could heal. From there he was transferred to the neurology floor, where he slowly improved. While there, his daughter Addison got to see him for the first time. Angie says, “When Addison saw her daddy for the very first time, she was relieved, happy she could hug him, and that he was alive.” Miraculously his legs were not broken and he was able to hold simple conversations and walk a few steps.    As the days went on, Brandon’s dad returned to Oregon, and the Inn at Renown became the family’s new home. “It was such a blessing to be so close to Brandon,” Angie observes. “It allowed us to relax a little being in the same building and let us settle in.”  The Comeback Brandon’s first memory after the accident was waking up in the Renown Rehabilitation Hospital, 16 days after his accident, not knowing where he was. (Brandon experienced amnesia due to his brain injury and doesn’t remember the days prior to his rehab stay.) He recalls looking around the hospital room and seeing the photos Angie posted of their family on the walls and wondering, “How did they get pictures of my family?” Immediately after learning that Brandon was awake, Angie raced back to the hospital, went outside his window and talked to Brandon on the phone, reassuring him that they were there for him and everything was going to be ok.  The next morning, he saw Addison for the first time that he remembered since his accident. They each put their hands to the hospital window “touching” each other in an emotional reunion. During the pandemic, each rehab hospital room was designated with an animal, so family and friends could visit outside safely.  Brandon was in the ‘moose’ room. “Whoever thought of that was a genius,” notes Brandon.  A new phase of Brandon’s recovery began at the rehab hospital. "He worked so hard while he was there,” Angie shares. With a minimum of three hours of daily therapy sessions, including speech, occupational and physical therapy. Angie participated in every aspect of his therapy, “I learned so much from the therapists; they included me in everything,” she recalls. “The compassion and patience they have is amazing. It takes a special type of person to do this job. I cannot say enough good things about the Renown Rehabilitation Hospital staff. If he would have been anywhere else, I’m not sure he would be alive,” she says.  It wasn’t easy. Brandon had a brain injury that required a bolt in his skull to relieve the pressure, and a broken left collar bone and left eye socket. His entire face had to be reconstructed. He remembers his face hurting and thinking he looked like Freddy Krueger. When he saw himself in the mirror for the first time, he was surprised to find he only looked thinner, with a gauze pad on his temple.   Prior to the accident, Brandon weighed 300 pounds, but had just finished a year-long fitness journey losing 119 pounds, by doing CrossFit and overhauling his diet. During rehab his weight dropped to 160 pounds and he was known as “the double portion” guy, eating extra food to gain weight.   Shaun Stewart, Therapeutic Recreational Therapist, recalls Brandon riding the recumbent cycle during his recovery. “I remember him saying he didn’t know if he was ever going to be able to ride a bike again and was appreciative when adaptive sports were discussed. He was very willing to participate and excited to be able to get on a bike again. He had a positive attitude and always was willing to get up and get back on the bike.”  Better Together Although Angie and Brandon were in a committed relationship for almost 11 years, they were not legally married at the time of his accident. “In our minds, our lives were perfect,” Angie asserts. “We had lived together for so long and have a child together.” However, because of COVID-19 restrictions, Angie had to lie and tell the medical staff that they were married so she could be by his side. When Brandon woke up from a coma, she told him, “No matter what I’m your wife.” He asked, “What do you mean, you are my wife?” After hearing Angie’s explanation, Brandon said, “Then, let’s do it.”   “We realized when faced with death that the benefits far outweighed the negatives in becoming husband and wife,” Angie discloses. “The rest is history.”  On Tuesday, August 18, 2020, Brandon and Angie were married underneath the trees behind the rehab hospital. Their family, friends and several staff members attended the ceremony. “I think that’s a first for us,” declares Dr. Gavin Williams. “I cleared him for capacity to make decisions, and we had a COVID-friendly wedding on our back lawn before he went back home to Oregon.”  The next day, Brandon officially left the hospital. “I felt good. Like ‘he’s gonna make it,’ but I was also scared,” mentions Angie. The family stayed in town for a couple of days to make sure everything was ok and then traveled home to Oregon.  Not Today Dr. Williams set Brandon’s recovery in the range of six months to two years. Brandon set the six-month mark as his goal and returned to work full time in just under that time. Then, in March 2021, he and Denton completed the David Goggins challenge run -- running 4 miles, every 4 hours for 48 hours. He completed the run with “not today” on the front of his shirt – a new motto.  After the race we wrote a letter of thanks to the Renown Rehabilitation Hospital staff. “I have learned doing ‘hard’ things on purpose is so important. I’ve said this a few times over the past few months after my crash, but I maintain it is the only reason I am still alive. When you do ‘hard’ things, your body (and more importantly your mind) is much better prepared for those unplanned times you face hardships. Maybe even a conversation with death itself, where I somehow had the strength to answer with a firm ‘not today’.”   Brandon describes the rehab hospital as, “A phenomenal facility. Everyone is happy to be there from the doctors, nurses and therapists, to the cleaning staff. Everyone was helpful and nice. They reinforced the positive aspects of my recovery and kept my head in a good spot.”    “Brandon was a great patient and he has made excellent recovery from his brain injury and multiple fractures,” Dr. Williams observes.  Life Goes On These days you can find the Ott family happily spending time with Addison’s new German Shepard puppy Density, camping at Crater Lake for Father’s Day and planning future adventures. Angie reflects, “Life is so precious and tomorrow is never promised.”  Since his crash, Brandon sold all of his motorcycles and now prefers biking on his own power. He even completed a 50-mile ride on July 8, 2021.  “Before this happened, I was on the judgmental side,” Brandon confesses. “This recovery led me to a place of compassion realizing everyone has their own struggles. I’m a much more caring person and am aware how fragile life is. I appreciate life a lot more.”

    Six Tips for Healthy Vision

    You might think worsening eyesight is inevitable as you age. But the truth is, there are easy things you can do to keep your eyes in tiptop shape for years to come. Clear vision is an essential part of overall health and there are daily habits we can adopt to keep our eyes seeing clearly. Mitchell Strominger, M.D., a registered ophthalmologist with Renown Health specializing in pediatric ophthalmology and neuro-ophthalmology, offers some everyday tips to keep you focused on eye health. Know your family’s vision history Look to your parents and grandparents for clues about the future of your eye health. Were they near-sighted at a young age? Do they have a history of eye crossing or a lazy eye? Did they develop glaucoma or macular degeneration (AMD)? Some genes have a strong association with eye health, specifically macular degeneration. AMD is a leading cause of blindness worldwide and the top cause of vision loss and blindness for Americans over 65, according to the Centers for Disease Control and Prevention (CDC). Also be aware certain medical conditions, such as diabetes, high blood pressure and liver disease, increase the risk of long-term eye problems. Receive regular vision screening exams Children under three need vision screening examinations by their pediatrician. A medical concern or family history warrants a comprehensive eye examination by a pediatric ophthalmologist (or general ophthalmologist or optometrist who has experience with children) per the American Academy of Ophthalmology (AAO). Vision screenings should occur every two years throughout adolescence unless pain, eye crossing, difficulty seeing the board or reading, or other eye concerns occur. In adulthood the risk of eye problems increases over age 40, so the AAO recommends regular dilated exams. When your pupils are dilated it allows your eye care professional a more accurate view of your retina and optic nerve, located in the back of your eye. This allows them to look for AMD or other eye problems. Eye diseases can be caught at their earliest stages during a dilated eye exam. “It is especially important to have your child evaluated as soon as possible if there is any concern for eye crossing or lazy eye,” says Dr. Strominger. “The visual system develops in the early years so intervention, whether it be glasses, eye patching to strengthen the vision in one eye, or eye muscle surgery is critical. Children at risk include those who are born premature, have development abnormalities, genetic disorders such as Downs syndrome, or a strong family history and should be evaluated at a young age. Often small problems are not clearly evident and cannot be picked up on routine screening examinations in the pediatrician’s office.” Block the sun, improve your vision Everyone knows not to stare directly into the sun, but indirect ultraviolet (UV) sun rays can cause serious harm over time. According to the National Eye Institute, sunglasses (or a protective UV tint) are a daily must to protect your eyes from: Cataracts (a clouding of the eye’s lens causing blurred vision) AMD - macular degeneration Pterygium (a tissue growth over the white surface of the eye that can cause astigmatism) Look for sunglasses which block 99 to 100 percent of both UVA and UVB rays. You still need sunglasses if your contact lenses block UV rays. Sun rays can damage your eyelids and other tissue not covered by contact lenses. Wearing sunglasses protects your entire eye area. Eat colorful meals Your eyes need vital nutrients to keep them healthy including vitamin C, zinc, beta carotene and copper. A diet filled with citrus fruits and a variety of vegetables provide these essential nutrients. Regularly filling up on fish high in omega-3 fatty acids (salmon, tuna, sardines) may reduce your risk of AMD and help maintain your eye health. Green, leafy vegetables (kale, spinach, collard greens) containing lutein and zeaxanthin are also a must for your dinner plate. Water your eyes Eyes, like the rest of your body, need plenty of water to prevent dehydration. Make sure you stay hydrated and steer clear of smoke to avoid dry eyes and irritation. Hours staring at the computer screen can also make your eyes feel dry or tired. Try regularly refreshing them with lubricating eye drops. Taking frequent computer breaks is also important. Follow the 20/20/20 rule: every 20 minutes, look at an object 20 feet away from you for 20 seconds. Eye makeup can also lead to dry eye as the glands at the base of the eyelashes may become clogged, causing dry eye. Make sure all eye makeup is hypoallergenic and is thoroughly removed with a gentle cleanser for the delicate eye area each day. Stop Smoking (or never start) Smoking is harmful to every part of your body, including your eyes. It's not only linked to cancer and heart disease but also cataracts, AMD, dry eye, optic nerve problems and many other problems. Smoking during pregnancy can also harm the eyes of the unborn child. If you currently smoke take steps to quit and your entire body will benefit.

    Cancer Nutrition - Helpful Tips to Support Your Loved One to Eat Healthy

    Eating healthy is a daily challenge for many, but for those with cancer it is an even harder struggle. A cancer diagnosis not only impacts those diagnosed, but family members and friends too. One key area of concern is making sure your loved one stays well by eating healthy food every day. Here are some essential cancer nutrition tips from Renown Health registered dietitians Jessica Blauenstein and Amy Laster. Help to Prepare Meals and Snacks for Daily Cancer Nutrition Make it grab and go. Easy-to-make meals help reduce the burden of having to cook and prepare food. Having easy to grab snacks on the counter or in the fridge can help ensure your loved one has access to those nutrients when needed. Sit outside of the kitchen. This allows your loved one to avoid cooking smells which can make them feel sick as a side effect of cancer treatments. Also try serving them cold foods such as sandwiches, cheese and crackers, or shakes which have a mild scent. Try drinkable meals. Some people with cancer find it easier to sip their calories over the course of 30 minutes to an hour. Consider smoothies or supplemental shakes such as Ensure Enlive or Boost Plus as snacks not meal replacements. A great foundation for a smoothie is a protein source (Greek yogurt, protein powder, nut butters or milk) with a carbohydrate (fruits, juice or berries). Add other ingredients as desired, such as spinach, kale, and ground flaxseed or chia seeds to give it more vitamins, minerals, and fiber. Snack Ideas for Those Undergoing Cancer Treatment The following ideas are both quick and easy to make for your loved one. Chicken or tuna salad with whole grain crackers or as a sandwich on whole grain bread Greek yogurt mixed with cereal, fruit and/or nuts Cottage cheese with banana, cinnamon and/or peanut butter Favorite fruit with 100% natural peanut or almond butter spread - Try peanut butter with bananas, apples, or even celery Their favorite veggies dipped in a salad dressing of your choice - For example, carrots with hummus or ranch dressing Cheese and whole grain crackers - Add tomato slices with a dash of oregano on top for more flavor Eggs scrambled with cheese, vegetables and/or salsa Peanut butter and jelly sandwich on whole grain bread A baked sweet potato with some favorite toppings Hard boiled eggs and/or egg salad with whole grain crackers, or as a sandwich on whole grain bread Oatmeal or cream of wheat prepared with milk, fruit and/or nuts Sometimes your loved one may not feel like eating or refuse to eat. Cook for Your Life ELLICSR Of course you may also consult a registered dietitian and/or the patients care team, if you have more cancer nutrition concerns. No Appetite? Assisting with Cancer Taste Changes Taste changes are common during cancer treatment. Patients experiencing these changes may not feel like eating, which can negatively impact their nutrition. Help your loved one overcome taste changes with these cancer nutrition strategies: Metallic or bitter taste in food - add something sweet such as maple syrup, honey or jelly. You may also try adding fat, such as a nut butter, avocado or regular butter. Pickles or vinegar could help with this too. A taste like cardboard - try adding salt and extra flavor to foods with seasonings and spices. Some examples are onion, garlic, chili powder, basil, oregano, rosemary, tarragon, barbecue sauce, mustard, ketchup, or mint. Lemon juice, citrus, vinegar, or pickles may also help with this as well Food tastes too sweet - try adding six drops of lemon or lime juice. Add small amounts until the sweetness is gone. Very salty taste - try adding ¼ teaspoon of lemon juice. Try plastic utensils instead of metal, especially if your loved one is struggling with foods tasting metallic. Dipping small bites of food into either lemon juice or vinegar can have a “palate cleansing” feel and may improve taste perception. This helps avoid getting tired of the flavor after a few bites. Try marinating food or meats in sweet fruit juices, salad dressings, or sweet-and-sour sauce. Other “palate cleansing” foods are lime juice, orange juice, mangos, lemongrass, parsley, cilantro, mint, ginger, basil, and pickled foods. Use aroma to make foods appealing, avoiding any smells that may cause nausea. In particular, cinnamon, nutmeg, cloves, coriander, cumin, ginger, and black pepper can add an aromatic flavor. Also include herbs such as oregano, rosemary and thyme. Think texture. Consider trying wafers, crunchy nut butters, carrots, celery, cucumbers, chips, rice crispies, corn flakes, crackers, panko crumbs, nuts, or seeds if you are able to chew and swallow them safely. Remember that patience with your loved one's changing appetite and tastes can be the strongest form of support you can provide. Nutrition Tips for Cancer Survivorship and Beyond If eating large meals is difficult for your loved one, encourage them to snack throughout the day. Aim for 4-6 snacks or small meals per day. Focus on consuming 2-3 protein-rich foods each day such as lean animal meats, fish, eggs, soy or a protein supplement with 20-30 grams of protein per serving. Eat a variety of brightly colored fruits and veggies. Aim to make half of each meal fruits and vegetables. Eat less than 18 oz. (cooked weight) of red meat per week. Limit cold cuts, bacon, sausage, and hot dogs. Avoid excess salt and saturated fats. Cut back on simple carbs. (i.e. desserts, candies, white bread/pastas, french fries, packaged foods, chips) Drink in moderation- if at all- one drink per day for women, two for men. Get enough vitamin D (through diet and/or supplement) Avoid tobacco of any kind. Keep a healthy weight and stay up on exercise. Other Ways to Help a Loved One with Cancer Assist with Chores Allow your loved one to relax as you help take care of chores around the house, such as cleaning, cooking and grocery shopping. This may reduce overall stress and can be helpful for those that get tired after their treatments. Physical activity can still be beneficial, so be sure to do an activity together that they enjoy and is approved by their doctor. Be Patient It can be very difficult to see a loved one go through cancer treatment. Remember to be patient with your loved one. There may be things we want them to do or eat that we know would be good for them, however, meet your loved one with grace and understanding. It is ultimately their decision on what to do, just be there to support and assist them as they go through this challenging time.
